August 22, 1935 – June 21, 2025

Jack R. Hazelwood, Sr., 89, passed away peacefully on Saturday, June 21, at Carroll County Memorial Hospital in Carrollton, Ky. after a lengthy illness.

He was the retired owner of Hazelwood Music, formerly Bluegrass Music, Upholstery, and TV Repair in Carrollton, Ky. and a classic country singer from his band Jack Hazelwood with His Ramblin' Cowboys. He was also one of the oldest video game players.

He is survived by his sons, Jack (Peggy) Hazelwood, Jr. and Cary (Danielle) Hazelwood; his grandchildren, Jacob Hazelwood and Jillian (Kyle) Macdonald; and numerous nieces, nephews, grandnieces and grandnephews.

He is preceded in death by his wife, Martha Ray Hazelwood; his five brothers, Arthur, Curtis, James A., Raymond, and C. Foster Hazelwood; his three sisters, Anna M. Kearnes, Dorothy Hyatt, and Gertrude Crank.

His wishes were to be cremated. No funeral services will be held.
