Amy was born in Madison, Jefferson County, IN to Glenn Allen Johnson and Vickie Sue Welch Johnson (Lewis). She was a member of the Family Worship Center and was a Cook and Server at Waffle House in Carrollton, KY.
She is survived by her husband - Jason Perkins of Owenton, KY, 4 sons - Exavier, Justice, Elijah and Trenton Perkins - Fairfield, OH, her mother and step-father - Vickie Sue and Mark Lewis - Sanders, KY, her father and step-mother - Glenn Allen and Vicky M. Johnson - Sparta, KY, 2 brothers - Jeffrey and Ricky Welch - Carrollton, KY, 1 step-sister - Angel Wente - Seymour, IN, 3 step-brothers - Joshua and Bradley Lewis - Sanders, KY and Allen Johnson - Sparta, KY., her maternal grandmother - Flora Gibson of Carrollton, KY.
She was preceded in death by 1 brother - Larry Dewayne Johnson and 1 half-brother - Glenn A. Johnson, Jr., maternal grandfather - Clifford Welch. materanl step-grandfather - Alton Gibson, paternal grandparents - Cannan & Eula Johnson.
Funeral Services were on Thursday - March 14, 2013 at Tandy-Eckler-Riley Funeral with Pastor Pat Butcher officiating the service. Burial was in the Dallasburg Masonic Cemetery in Wheatley, KY.
Memorial gifts may be made to the family to assist with expenses in care of the funeral home.
