He was born on December 4, 1964 to the late Donald and June Davis Lewellyn. He is survived by his nephew, Kevin Lewellyn, Jr. of Carrollton, KY.
He is preceded in death by his parents and a brother, Kevin R. Lewellyn, Sr.
